Complete the BPCF Small Grant Request Form – include as part of your grant request:

Provide a list of all the materials and the total cost of the materials needed for your project.
Explain how you will fund your project other than the BPCF Grant, including how much money you have raised so far.
Give a target date for the completion of your project.
How will you acknowledge the funding provided by the BPCF, if approved?
